2021-2022 School Year

Open House information is forthcoming.

The school will begin accepting applications for the 2021-2022 school year for students in Kindergarten-12th grade beginning May 1st, 2021, and ongoing.

Paperwork Required for Application to Grades Kindergarten through 12:

Placement Test (Kindergarten-Grade 12)
Copy of Report Card grades in all subject areas from the past two years to present grade (or appropriate marking period)
Copy of Standardized Test Scores from past two years to present
ORIGINAL Birth Certificate (FLPS will photocopy) (Kindergarten – Grade 12)
ORIGINAL Health Records must be submitted on the State of Florida approved forms (HRS 680 and HRS 3040) Current health records, original 680/340 must be on file with the school prior to student beginning school/classes. (Kindergarten – Grade 12) students coming from out of state must have immunizations transferred to the approved forms (HRS 680 AND HRS 3040) by a Florida doctor or the Florida Health Department once have moved to Florida. Their pediatrician will have the HRS 680 and HRS 3040 forms for the health records.
